2017-2018 Student Registration
Summer 2017 Student Registration Dates

If you need to register a student for the upcoming 2017-2018 school year, please contact your resident school beginning August 9,  2017 to make a registration appointment

Please have all required documentation as we will only be scheduling and accepting complete registrations at this time.  Please refer to the Student Registration Instructions below prior to scheduling an appointment. 


The general registration period for the 2017-2018 school year will begin on January 25, 2017 and will conclude on February 24.  Registration for new students entering the Millbrae School District and students entering Transitional Kindergarten and Kindergarten for the 2017-2018 school year will require an appointment.   You are required to make an appointment with your home school to submit the required documents and will then be provided instructions to register your new student online. Please follow the registration instructions below to successfully register your student.

Transitional Kindergarten – All students who turn five (5) years old between September 2 – Dec. 2, 2017 (inclusive) may enroll in Transitional Kindergarten for the upcoming 2017-2018 school year.  


1.   Please print and complete the 2017-2018 Student Data Sheet below:

2.   The following documents MUST BE PROVIDED:

  • Student Birth Certificate (original or certified copy)
  • Parent/Guardian Identification (Actual Driver’s License or Passport)
  • Residency Verification – 2 documents are required with parent/Guardian Name and Address listed on both (please click here for additional Residency Verification Information)

Homeowner or Renter/Lessee - please provide either (1) Grant Deed or (2) Property Tax Bill or (3) Original Rental or Lease Agreement AND current PG&E Bill or PG&E Confirmation of Service or current Water Department Bill

     □Grant Deed/Property Tax/Lease Agreement
     □Utility Bill

  • Current Immunization Records – Original Immunization Card (yellow card) or Health Provider Immunization Report

  • Current TB results taken within the last 12 months  (if registering for the first time in a California Public School, if student is from out of country or state, or first time registering for Transitional Kindergarten or Kindergarten)
  • Proof of Tdap Immunization (7th & 8th Grade registrations only)

If applicable:

Current IEP or 504 Plan
Expulsion Order
Custody Documents

3.   Please contact your resident school beginning August 9, 2017 to schedule a registration appointment.  For this appointment, you must provide the completed Student Data Sheet and all required documentation as listed above in order to successfully begin the registration process. 4.   When all registration documentation is received, you will receive instructions to compete the process and register your student online.  Please register your student as soon as possible.  All students will be assiged to their resident school based on their home address and classroom space availability.  If there is a lack of classroom space, your student will be placed on a waiting list for your resident school but, in the meantime,  will be assigned to another school within the Millbrae School District with the available classroom space.


For the remainder of 2017-2018, please contact your home school to make an appointment to register your student.  You will need to provide all required documentation in order to register your student sucessfully.


Immunization Law SB 277

Under new law known as Senate Bill 277, beginning January 1, 2016, exemptions based on personal beliefs will no longer be an option for the vaccines that are currently required for entry into a California School.  Personal Belief Exemptions on file for a child already attending a California School will remain valid until a child reaches the next immunization checkpoint at Kindergarten (including Transitional Kindergarten) or 7th Grade. For more information about Senate Bill 277, please visit Shots for School at:
